Advance Health Care Directives
A specially trained Long-Term Care Ombudsman will
assist residents living in skilled nursing facilities
throughout Contra Costa County who wish to execute
an Advance Health Care Directive. An Advance
Health Care Directive is the best way to make sure
that your health care wishes are known and
considered if for any reason you are unable to speak
for yourself.
We will provide the forms and will explain each step
of the process so that the resident's wishes are
accurately reflected in the document.
